Passport Services (by appointment only)


The United States Department of State Passport Services has designated the City of Dublin as an authorized Passport Acceptance Office. U.S. Passports are issued only to U.S. Citizens or Nationals. Each person must obtain his or her own passport.

Passport applications are accepted by appointment only. Appointments are for one (1) passport applicant up to a maximum of six (6) passport applicants per appointment. No walk-in service available.

If you have a life-or-death emergency need or urgent travel (non-emergency), please call the Department of State at 1-877-487-2778 to schedule an appointment. More information about Life-or-Death Emergency Service and Urgent Travel Service is available on the Department of State website. 

The City does accept applications for adult renewals that are going to expire or have expired within the last five (5) years. Please see the Department of State information about renewing by mail:

Before your Appointment:

  1. Completed Application [unsigned application (Form DS-11) completed single-sided in black ink]:
    • Social Security Number is required on the application
    • Forms can be found online at https://eforms.state.gov/(When printing an electronic version of the application, select 'Fit to Paper' or 'Actual Size' under printing preferences. Margins must be no larger than 1/2 an inch on each side) Print Settings for Passport Applications
    • Do not sign your application; the application must be signed in the presence of the Passport Acceptance Agent
  2. Passport Photos: Please bring passport photos with you to your appointment. The City of Dublin DOES NOT offer passport photo services. Do not staple passport photo to application. Passport photo requirements can be found online at https://travel.state.gov/content/travel/en/passports/how-apply/photos.html.

  3. Evidence of U.S. Citizenship (Please bring the original and a photocopy of one of the following):
    • Certified Birth Certificate
      • Minors under age 16 must submit Certified Birth Certificate to show parental relationship
      • Neither Abstract (simplified) Birth Certificates nor Hospital Birth Certificates are acceptable
    • Consular Report of Birth Abroad
    • Certificate of Naturalization
    • Previously issued undamaged U.S. Passport
      • Applicable for adult applicants not eligible to renew their passport by mail
    • If the applicant’s name does not match the name on their citizenship evidence, a name change has occurred and must be supported by additional acceptable documentation, such as a certified marriage certificate or court order.
  4. Photo Identification (Please bring the original and a photocopy of one of the following):
    • Valid Driver's License
      • Note: Out of State Identifications require a secondary form of identification
    • Current Valid Passport
    • Valid Military Identification
  5. All Applicants must be present:
    • All applicants, including children, must be present during application process. For children under 16, both parents (with identification) must be present at the time of applying. If both parents are not able to appear in person, call before your appointment for additional requirements.
