|Massage Establishment Permit Application
|Massage Establishments must obtain permits from Dublin Police Services before they begin operating within City limits. This permit application form is required for new permits, permit renewals, permit amenments, and ownership changes. For a new permit, submit the application with the $464.00 fee; the cost of a renewal is $296.00.
|Massage Therapist Registration
|Massage Therapists must be registered with Dublin Police Services before they begin operating within City limits. To register, the massage establishment owner or massage therapist must submit the application with the required supporting documents.
|Massage Establishment Permit Application Amendment - Add/Remove/Renew Employees
|The City of Dublin requires that owners of massage establishments register all their employees with Dublin Police Services and promptly report any employee changes. The establishment owner must submit this form to Dublin Police Services to report any new additions or removals among their employees, as compared to the list of employees provided in the Massage Establishment Permit Application. Additionally, owners should utilize this form when submitting a CAMTC renewal/recertification application for massage therapists.
