Each of the following documents is required as part of the registration process for your child's acceptance into the Dublin Preschool Program. Please complete each document in its entirety.  Return all the documents together including a copy of your child's birth certificate, a copy of your child's current immunization record, proof of Dublin residency (Dublin Residents Only) along with the student file checklist prior to July 1st to the Shannon Community Center Office located at 11600 Shannon Avenue, Dublin, CA 94568. YOUR CHILD WILL NOT BE FORMALLY ACCEPTED INTO THE PROGRAM UNTIL ALL PROVISIONS HAVE BEEN SATISFIED.
